MINNEAPOLIS, Minnesota—Health Dimensions Group (HDG), a senior living, post-acute and long-term care management and consulting firm based in Minneapolis, Minnesota, is celebrating its 25th anniversary in 2025.

Founded in 2000, HDG said its mission is to provide care, service and consulting. HDG manages 52 senior living and care communities in nine states, caring for more than 3,000 residents. Serving a broad range of clients in the aging services sector across all 50 states, HDG’s consulting division provides customized solutions tailored to each client’s needs from strategy development, operational performance improvement and interim leadership to turnaround support.
