CLEARWATER, Fla. (June 5, 2018)—Synzi, a virtual care company, has announced that Trilogy Home Healthcare (Trilogy), a Medicare-certified home health care agency, has selected its virtual care communication platform to improve patient management, engagement and care.

Trilogy works with home health nurses, medical directors, community physicians and other staff to provide in-home clinical care to patients in the state of Florida. The organization was looking for a virtual technology solution to enhance staff and patient communications, while also offering patients’ out-of-town family members a convenient way to become involved in their loved ones’ care, thus alleviating the guilt often associated with not living close enough to provide hands-on support.

Trilogy selected Synzi’s virtual care platform for its comprehensive video, email, text and secure messaging capabilities. Moreover, Trilogy will be able to better serve its diverse patient population by utilizing the platform’s unique ability to provide direct access to a video interpreter for limited English proficiency (LEP) and Deaf/Hard-of-Hearing patients.

Trilogy’s initial platform deployment is being executed in late May and early June. The initial focus will be on patients who own and have a comfort level with smart technology. A secondary focus will be on, but not limited to, patients who are dealing with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and diabetes.

Synzi’s communication platform leverages everyday devices such as smartphones, tablets and laptops to automate and optimize workflows to help home health agencies provide cost-effective, patient-centric care. Trilogy will also use the Synzi platform to schedule a series of appointments with their patients and send appointment reminders via phone, secure messaging, or email—depending on their patients’ preferred communication channels. As needed, the appointment reminders can be translated into the patient’s primary or preferred language.
