This partnership will allow KanTime’s home health, hospice, palliative & homecare providers to access & share patients’ advance care plans

DALLAS—MyDirectives, a provider of digital advance care planning (ACP) solutions, and KanTime Healthcare Software, a provider of enterprise software for post-acute care, announced a strategic partnership to embed MyDirectives for Clinicians directly into the KanTime electronic health record. This integration will allow KanTime’s home health, hospice, palliative and homecare providers to access and share patients’ advance care plans, delivering goal-concordant care, improving patient and family satisfaction and avoiding unwanted treatments or interventions.

With the integration underway and full availability expected in the coming weeks, KanTime clients can gain instant access to ACP documents, including advance directives, portable orders for life-sustaining treatment forms, Five Wishes and mental health advance directives, all without leaving the KanTime workflow. By ensuring these documents are available across care settings, clinicians can honor patient preferences during transitions of care—especially critical in emergency department and emergency medical services scenarios.

Through this partnership, KanTime said its users will also benefit from anytime, anywhere access to ACP documents via the A|D Vault Exchange, MyDirectives’ propriety nationwide registry. Built on a HITRUST-certified and SOC 2 Type II–audited platform, the A|D Vault Exchange ensures that critical documents can be securely accessed and shared across health care organizations, health information exchanges and emergency settings, no matter where.
