This partnership will allow KanTime’s providers to access & share patients’ advance care plans

DALLAS—MyDirectives, a provider of digital advance care planning (ACP) solutions, and KanTime Healthcare Software, a provider of enterprise software for post-acute care, announced a partnership to embed MyDirectives for Clinicians directly into the KanTime electronic health record. This partnership will allow KanTime’s home health, hospice, palliative and homecare providers to access and share patients’ advance care plans, delivering care, improving patient and family satisfaction and avoiding unwanted treatments or interventions.

With the integration underway and full availability expected in the coming weeks, KanTime clients can gain instant access to  advanced care planning (ACP) documents, including advance directives, physician order for life-sustaining treatment (POLST) forms, Five Wishes and mental health advance directives (MHADs), all without leaving the KanTime workflow. By ensuring these documents are available across care settings, clinicians can honor patient preferences during transitions of care.

Through this partnership, KanTime users will also benefit from anytime, anywhere access to ACP documents via the A|D Vault Exchange, MyDirectives’ propriety nationwide registry.
