The program is designed to help members recover at home & prevent hospital readmission

PHILADELPHIA—Medical Guardian, a provider of medical alert technology and remote monitoring, and Mom's Meals, a food as medicine solutions provider, are bringing targeted recovery support to aging adults recently discharged from the hospital.

Through this new benefit, Medical Guardian will cover the cost of Mom's Meals for its private-pay members who are recovering at home after a hospitalization resulting from an emergency dispatch through their medical alert device. Registered dietitians and professional chefs develop these medically tailored meals to meet a range of dietary needs, including diabetes, heart disease, renal conditions and gluten intolerance. The meals are packaged for storage in the refrigerator for at least 14 days from delivery directly to members' homes to help promote independence, ease and dignity during recovery.

Hospital readmissions remain a significant challenge for health care systems, particularly among older adults. Malnutrition is a key contributor—patients who are malnourished during hospitalization are 54% more likely to be readmitted within 30 days. Despite its impact, up to half of hospitalized patients are at risk for malnutrition, yet it often goes unnoticed and untreated.

As part of Medical Guardian's ongoing member engagement and care, the company's wellness team checks in on members following a hospital stay. For those eligible, the team will assist in redeeming the covered meal benefit, walking members through how to select their meals and schedule delivery either online or by phone.
