Hospice Tools EMR was awarded the accreditation for the second year in a row.

SKOKIE, Illinois—Hospice Tools is pleased to announce that Community Health Accreditation Partner (CHAP), has re-verified Hospice Tools EMR, awarding it CHAP Verified status for the second consecutive time. This designation is granted to tools and resources used by community-based care providers that, through rigorous inspection, show no roadblocks to executing the CHAP Standards of Excellence and aim to improve the delivery of high quality care.

CHAP said it conducted a thorough product survey to verify that Hospice Tools EMR allows organizations to meet the intent of CHAP's applicable standards.

The CHAP review ensures that Hospice Tools EMR empowers agencies to follow best practices for clinical documentation, operations and reporting. The product survey mapped out the life cycle of a hospice patient including clinical workflows from intake through discharge and bereavement, quality reporting, human resources, operational workflows and mobile charting.

According to CHAP, Hospice Tools showcased the ability to adapt and navigate specific scenarios with proprietary hospice-centric features including:

  • Permissions-based views and workflows for role-specific efficiency and compliance
