NEW YORK (September 15, 2016)—Hometeam, a new approach to homecare that brings more independence, joy and vibrancy to older adults—and peace of mind to families—announced that it has partnered with Fox Rehabilitation, a professional private practice of full-time physical, occupational and speech therapists in ten states on the East Coast.

Hometeam and Fox are partnering to create an innovative model of care that supports the benefits of physical, occupational and speech therapy in the patient’s home after a skilled therapy episode through improving adherence to therapist-prescribed home exercise programs. This unique collaboration between clinical and non-clinical teams will promote optimal living through functional, cognitive and social engagement to enable older adults to successfully age-in-place. Fox clinicians perform geriatric house calls and identify individuals in need of additional help with activities of daily living, companion care and compliance to exercise programs designed by the Fox clinical team after discharge. The partnership also provides access for Hometeam's expanding client population to the evidence-based and clinically excellent therapy services and programs at Fox.
