NEW YORK (September 15, 2016)—Hometeam, a new approach to homecare that brings more independence, joy and vibrancy to older adults—and peace of mind to families—announced that it has partnered with Fox Rehabilitation, a professional private practice of full-time physical, occupational and speech therapists in ten states on the East Coast.

Hometeam and Fox are partnering to create an innovative model of care that supports the benefits of physical, occupational and speech therapy in the patient’s home after a skilled therapy episode through improving adherence to therapist-prescribed home exercise programs. This unique collaboration between clinical and non-clinical teams will promote optimal living through functional, cognitive and social engagement to enable older adults to successfully age-in-place. Fox clinicians perform geriatric house calls and identify individuals in need of additional help with activities of daily living, companion care and compliance to exercise programs designed by the Fox clinical team after discharge. The partnership also provides access for Hometeam's expanding client population to the evidence-based and clinically excellent therapy services and programs at Fox.

“We are excited to be partnering with a leading geriatric rehabilitation provider that has an exemplary presence on the East Coast,” said Josh Bruno, founder and CEO of Hometeam. “Our relationship is logical and linear. Hometeam works to provide excellent care for older adults in their homes, and Fox Rehabilitation will enhance that care with its tremendous resources, including more than 900 clinicians. Fox’s services will enhance the array of specialties we already provide and are consistent with our mission to provide beautiful days for everyone in our care.”

The partnership will foster an effortless continuum of care in the comfort of the older adult’s own home. Fox's clinical evidence-based programming to address functional limitations, fall risk and safety in the home will be further enabled by Hometeam caregivers as they help the patient population be in compliance between Fox clinical visits and after discharge to maintain benefits of therapy interventions. The integration of physical, occupational and speech therapy will continue to help lower hospital readmissions, decrease falls, improve function for those living with chronic conditions or recovering from injury, and result in a better quality of life.

“We view this partnership as a significant step in our strategy to proactively address health care reform, and to work with other providers to find clinically appropriate collaborations and innovative means to deliver health care. Clinically, this is a less expensive approach to better outcomes. This is a unique and novel approach focused on decreasing hospitalizations, decreasing falls and allowing our older adult clients to remain in their homes longer. It is extremely timely with health care reform, ACO, bundles and collaborates that must occur," said Dr. Tim Fox, founder and CEO of Fox. “The value of the connectivity and collaboration of this size with the ability to leverage service excellence and technology to further enhance the level of care we are providing will be a real benefit to our partners, our patients and their families.”

Hometeam’s proprietary software matches caregivers with patients not just by medical specialization and needs, but also through shared hobbies, interests, personalities and past experiences. Hometeam provides each home with a tablet so caregivers, doctors and family can check the status of the older adult on a routine basis. Fox rehabilitation specialists can also access the software on this tablet, allowing them to monitor and manage the plan of care in place.
