CONCORD, New Hampshire—Granite VNA, New Hampshire’s non-profit home health and hospice agency, has promoted Nicholas Heafield to homecare manager and hired Tammy Martel as hospice community manager. Heafield previously served as a staff physical therapist and Martel served as a community hospice nurse. Martel was hired from Presidential Oaks in Concord.
Granite VNA is a not-for-profit home health and hospice agency providing care and wellness programs to residents of 72 communities throughout the greater Capital region and central New Hampshire and at Hospice House in Concord.
In his new role, Heafield, will manage clinical business operations and patient care services. Granite VNA said he will guide the homecare services team to ensure they have the support and tools they need to provide the best possible care for every patient.
“Having worked alongside Nick for four years, I have great confidence in his ability to step into this role,” said Granite VNA Senior Director of Patient Care Services Cindy Bergeron. “His promotion is well-deserved and represents our commitment to creating meaningful career pathways that provide team members with opportunities to develop their skills and advance professionally.”
Heafield holds a bachelor’s degree in exercise science from Castleton University and a doctor of physical therapy degree from Franklin Pierce University.
In Martel’s new role, she will support the clinical operations of the Community Hospice Program through organizing the hospice community team, advising in patient goal setting and supervising the home hospice nursing team.
“Tammy’s leadership, communication, planning and clinical skills make her an excellent fit for this role,” said Bergeron. “Her dedication to our patients and her peers is evident in everything she does.”
Since leaving Granite VNA in 2022, Martel has served as a nursing director and short-term rehabilitation unit manager at Presidential Oaks in Concord. Martel holds an Associate of Science in health science from New Hampshire Technical Institute.
