ROOSEVELT, Utah (September 9, 2022)--Canyon Home Care & Hospice, a leading home health and hospice provider in the Intermountain West, has announced the acquisition of Uintah Basin Home Health and Hospice, a division of the Home Care Services of Uintah Basin Healthcare.

Working in collaboration with Uintah Basin Medical Center, Uintah Basin Home Health and Hospice has demonstrated a proven track record of providing greater access to home health and hospice care in the home, effectively reducing emergent and long-term institutionalization for home health care patients, and improving health outcomes for all patients serviced by Uintah Basin Home Health and Hospice.

"As of Sept. 1, 2022, Canyon Home Care & Hospice takes responsibility for our home health & hospice services," said UBH President and CEO Jim Marshall. "Our patients' last day of service with UBH home health and hospice services is Aug. 31, 2022. (Please note that this does not include our durable medical equipment services.)
