FORT COLLINS—Canyon Home Care & Hospice, a full-service home health care provider, announced that it has acquired Columbine Poudre Home Care and Bloom at Home, effective Feb. 28, 2026. This acquisition will expand comprehensive care services to more communities throughout northern Colorado.
"Canyon Home Care & Hospice is so similar to Columbine Health that it was just a great fit from day one," said Holly Johnson, former administrator for Columbine Home Care who has joined Canyon Home Care & Hospice as an administrator. "This isn't the end of what Columbine Home Care built; it's the next step. With Canyon providing Hospice care, we can now offer a continuum of care that is needed in northern Colorado."
For seven years, Canyon Home Care & Hospice has served communities across the state of Colorado including Fort Collins, Loveland, Berthoud, Windsor, Greeley, Evans, Fort Morgan, Fort Lupton, Firestone, Frederick and Denver. With this acquisition, the organization said it hopes to strengthen its ability to provide a full spectrum of services, which include medical and non-medical homecare, hospice care, infusion services and medical equipment.
The companies assured atients of Columbine Poudre Home Care and Bloom at Home the acquisition would be a seamless transition. Clinical teams, care plans and scheduled visits remain unchanged moving forward. To ensure continuity of care, all Columbine Home Care employees were offered positions within Canyon Home Care & Hospice.
"Over the past years, when we couldn't accept a new care patient, Columbine Home Care was the first we called because of how similar they were to our practices, ethics and values," said Rae Roberts, administrator with Canyon Home Care & Hospice. "Now that we've joined, our strength is even greater. This transition is administrative in nature and allows us to strengthen operations and expand support resources."
