TEMPE, Arizona—Levine Leichtman Capital Partners (LLCP), a Los Angeles-based private equity firm, announced it has partnered with management to acquire SYNERGY HomeCare, a provider of in-home care services, from private equity firm NexPhase Capital. SYNERGY HomeCare said it will continue to be run by its current executive team and led by its current CEO, Charlie Young.

TYLER, Texas—Family Tree Private Care, a private care operator in the U.S., announced its merger with and into Choice Health at Home, a home health, hospice, personal care and rehabilitation services operator. The merger aims to establish Choice Health at Home as a nationwide operator that offers a full continuum of in-home care to patients.

WINTER PARK, Florida—AdventHealth, a health care services provider, announced the launch of a hospital at home program that aims to provide its Central Florida patients with hospital-level acute health care in their homes. The program, which launched at AdventHealth Winter Park, Florida, aims to enable patients who would have required an inpatient stay to receive care at home.

The company said this expansion aims to bring personalized care to the senior community in Pittsburgh and focus on a peer-to-peer caregiving model.

ENCINITAS, California—A Place At Home, a senior-focused homecare provider franchise, announced the opening of its fourth California location, A Place At Home Encinitas. Local couple Joscelyn Perez-Van Valzah and Sam Van Valzah opened the location, through which they intend to serve North County San Diego.

DENVER—DispatchHealth, an in-home medical care provider, announced it has received the Accreditation Commission for Health Care's (ACHC) new in-home hospital care accreditation for its care services. The company said this recognition aims to further the standard of hospital-alternative homecare that it provides to patients.

LOS ANGELES—Seen Health, a health care organization focusing on aging-in-place services and culturally-focused care models, announced it has invested $22 million in funding to expand access to its culturally-focused care centers for eligible seniors. The company said the first center is set to open in California and will be built upon the Program of All-Inclusive Care for the Elderly (PACE) model.

NASHVILLE, Tennessee—Biofourmis, a technology-enabled care delivery company, has partnered with Lee Health, a not-for-profit health system, to launch a new hospital at home program. The home-based hospital care model aims to expand the companies’ remote patient monitoring (RPM) programs and support patients across the care continuum.

CHICAGO— Help at Home, a national provider of in-home personal care services, announced the acquisition of Caregiver Services Inc. (CSI), a caregiver referral services provider. Help at Home said the acquisition aims to expand its homecare presence in Florida, as well as its Medicaid managed care programs and homecare services for aging seniors.

RENTON, Washington & BRENTWOOD, Tennessee—Providence, a not-for-profit health system serving the Western United States, and Compassus, a national provider of integrated home-based care services, announced an agreement to form a joint venture in home health, hospice, community-based palliative care and private duty caregiving services.

HOBOKEN, New Jersey—Health Recovery Solutions (HRS), a remote patient monitoring (RPM) company, announced its partnership with Smart Meter, which aims to enhance the company’s “PatientConnect” RPM solution. The PatientConnect system offers cellular measurement device as an alternative to traditional Bluetooth-based solutions.
