WellSky announced it has integrated its CarePort Community Referral Network into the Epic Toolbox category for Community Resource Networks

OVERLAND PARK, Kansas—WellSky, a health and community care technology and services company, announced its WellSky CarePort Community Referral Network solution is now part of the Epic Toolbox category for Community Resource Network (CRN). Epic users can now initiate, send and secure referrals directly to WellSky’s network of more than 130,000 post-acute and community-based providers from within their existing Epic interface.

Epic CRN clients who select the WellSky CarePort Community Referral Network solution gain access to a suite of capabilities designed to streamline care transitions and support whole-person care. Through a standardized, electronic referral process embedded directly into the Epic Case Management application, care teams can connect patients with a range of providers who can accept and manage referrals online, including skilled nursing facilities, home health agencies, inpatient rehabilitation facilities, long-term acute care hospitals, transportation services, durable medical equipment suppliers and social and behavioral health organizations.

Care teams also have access to a regularly updated, real-time provider directory where referrals are accepted with an average response time of 24 minutes. By leveraging the WellSky CarePort Community Referral Network solution, care teams can initiate referrals directly within their native Epic environment.
