SAN ANTONIO—Home Instead San Antonio, a provider of in-home care for older adults, announced the expansion of its core service offering with the introduction of nurse-directed care, providing another level of support for older adults with evolving care needs at home.
Designed for older adults recovering from surgery, managing chronic conditions or navigating more involved daily care routines, nurse-directed care combines guidance from a registered nurse with hands-on help from specially trained Home Instead Care Pros. Enhanced services may include medication management, oxygen therapy, diabetes support, mobility assistance, catheter and incontinence care, vital sign monitoring, ostomy care, wound care and other advanced care routines at home.
“Families want solutions that help their loved ones stay at home safely, even as care needs become more complex,” said Matt Moore, territory manager for Home Instead San Antonio. “Expanding into nurse-directed care allows us to provide an added layer of support while continuing to deliver the compassionate, high quality care families know and trust from Home Instead.”
Nurse-directed care begins with an assessment from a registered nurse who works with families to develop a custom care plan and provides oversight and direction to specially trained "Care Pros" delivering care in the home. Like all Home Instead care, the service is built around each individual’s needs, routines and goals while providing more day-to-day consistency.
“Families are often trying to navigate multiple providers, changing care instructions and evolving medical needs all at once, which can quickly become overwhelming,” said Jules Stracquadanio, registered nurse and care quality manager at Home Instead. “Our nurse-directed care service gives families access to a registered nurse who can help guide those conversations, support communication across care teams and help families feel more confident navigating care at home.”
In addition to nurse-directed care, Home Instead San Antonio provides a range of in-home support services including companionship, personal care, meal preparation, transportation, memory care and other services.
