FORT LAUDERDALE, Florida—Chambers Home Health & Hospice, a provider of home health and hospice services across Northeast Texas, announced it has entered into a partnership with Lucent Health Group (LHG).

The organization operates through its three agencies, Chambers Home Health, Chambers Hospice and Healthcare Associates LLC.
