Compassus Becomes First Hospice to Receive Age-Friendly Care at Home Certification

BRENTWOOD, Tennesse—Compassus, a national provider of home-based health care services, recently received the first Age-Friendly Care at Home certification for hospice in the country, awarded by Community Health Accreditation Partners. CHAP is a nationally approved accrediting organization that assesses the quality of health care providers. By achieving this certification, Compassus has demonstrated the integration and application of key principles and practices in providing high-quality hospice care for older adults.
 
CHAP developed the Age-Friendly Care at Home certification as an extension of the Age-Friendly Health Systems program, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and Catholic Health Association of the United States. To be certified, a provider must support a care model that follows four evidence-based elements of high-quality care, known as the “4Ms”—what matters most, medication, mentation and mobility.
