NASHVILLE, Tennessee—Anderson Family Care, an end-of-life care organization, announced its official launch to bridge the gap in how patients, caregivers and healthcare providers experience the final chapter.
Founded to address the industry challenge of late hospice referrals and fragmented provider communication, Anderson Family Care serves as the parent company of an integrated, dual-brand model offering advanced technology navigation, earlier goals-of-care conversations as well as high-touch, bedside care delivery. Counsel Care
A value-based identification and navigation platform designed for risk-bearing entities and primary care providers. Utilizing its proprietary algorithm, HospiceIQ, Counsel Care identifies appropriate patients earlier. The data insights are paired with trained “Benefits Counselors” who have the capacity to conduct unhurried, at-length family goals-of-care discussions at zero cost to the patient, while relieving clinical and administrative burdens from primary care providers.

A hospice care operating model built on community-rooted leadership. Anderson partners with local operators, granting local leaders bedside and operational decision-making authority rather than mandating from the corporate office. Closing a Gap
