HarmonyCares, Arbital Health Partner to Scale Value-Based Care for In-Home Services

SAN FRANCISCO and TROY, Michigan—Arbital Health, a health care technology and actuarial company that centralizes, measures and adjudicates value-based care contracts at scale, announced its partnership with HarmonyCares, a value-based care provider of in-home primary care services for people with complex health care needs. 

HarmonyCares said collaboration will enable it to streamline the reconciliation of value-based care contracts, optimize contract performance monitoring and deliver actionable insights critical to improving senior care outcomes across its value-based arrangements.

For more than 30 years, HarmonyCares has delivered compassionate care to seniors with complex health care and social needs in the comfort of their homes. Today, HarmonyCares supports more than 70,000 patients across 17 states through value-based care partnerships with Medicare Advantage plans and Medicare ACO programs. Together Arbital Health and HarmonyCares aim to drive better outcomes and efficiencies in delivering care to vulnerable populations.
