NEW YORK (April 27, 2015)—AMC Health, a leading provider of 'real-time' patient management solutions, will showcase advances in monitored care that are transforming chronic care and population health management at the upcoming ATA 2015 Conference, May 2-5 at the Los Angeles Convention Center.

AMC Health will share information about how their innovative programs, solutions and services are advancing monitored care to help transform and personalize healthcare delivery at Booth #2116 and during their joint presentation with Geisinger Health Plan (GHP) in Room #406AB on Tuesday, May 5th from 11 a.m. to noon. AMC Health and GHP will co-present a session entitled, "Using IVR and Telemonitoring to Lower Hospital Admissions, Readmissions and Costs for Heart Failure (HF) Patients." They will share results of a study that evaluated the impact of adding Bluetooth-enabled biometric telemonitoring and symptom/behavioral data capture via Interactive Voice Response (IVR) to GHP's highly regarded case management program for members previously diagnosed with HF.

The carefully crafted study analyzed 541 patients, who participated in the program for an average of two years, making it one of the longest, largest, and most comprehensive heart failure telehealth evaluations to date. This rigorous, peer-reviewed study is the first to accurately state dollar savings in clear terms and to do so with a large population tracked over two years. Some of the results from this landmark study, published in Population Health Management (Dec 2014), include: Patients on the program had a 23 percent lower probability of ending up in the hospital; Patients on the program who had been hospitalized had a 44 percent reduced probability of re-hospitalization within 30 days; Overall costs for patients participating in the program were 11.3 percent lower than the control group, which is a statistically significant reduction; and Net savings of nearly $2.7 million over two years for 541 patients translated to a 3.3-to-1 program ROI, which means that more than $3 were saved for every dollar spent.

Experts believe that this telehealth program, when added to heart failure case management operations that are not as advanced as Geisinger's, would deliver even higher ROIs. AMC Health and Geisinger Health Plan will also provide attendees with a better understanding of the processes, tools, training, resources and personnel required to implement the program. AMC Health's expertise in customizing and implementing chronic care management programs has contributed to significant improvements in health outcomes and reductions in costs of care that have been validated in peer-reviewed publications.
